DOCUMENTATION REQUIREMENTS / ANNEXES
(Please do not leave any blank information. Indicate N/A if information is not applicable)

A. GENERAL ASPECT
  1. Company General Information
    This should cover only a brief description of the company's mission and vision, history of excellence, nature of business, capabilities, expertise and other pertinent information that you feel will help us determine the qualifications and expertise of your firm as they would apply to the need of SKI as General Contractor. Please use a maximum of 2 pages only for this section.


B. LEGAL ASPECT
  1. Certified True Copy of SEC / DTI Registration
    To include any of the following applicable certified true copy documents:
        a. Certificate of Business Name Registration (if applicant is a Sole Proprietorship)
        b. Deed of Articles of Partnership Agreement (if applicant is a Partnership)
        c. Articles of Incorporation and By-Laws (if applicant is a Corporation)
  2. Certified True Copy of BIR Certificate of Registration
  3. Certified True Copy of Business Permit
  4. Certified True Copy of Mayor's Permit
  5. Certified True Copy of valid PCAB License (if applicable)


C. TECHNICAL ASPECT
  1. Organizational Chart (to include description of duties and responsibilities)
  2. Technical Qualification of Products (brochures) - for technical suppliers only
  3. List of Completed and On-Going Projects
  4. List of Key Technical Personnel (to include a brief statement of their qualification)
  5. List of Major Owned Equipment and Machineries
  6. List of Subcontractors and Suppliers
  7. Certified True Copy of Certificate of Project Completion issued by Owner.


D. FINANCIAL ASPECT
  1. Certified True Copy of Audited Financial Statement for the last 3 years.
  2. Certified True Copy of Income Tax Return for the last 3 years.
  3. Certificate of Bank Deposit or Certificate of Bank Recommendation describing the Applicant's financial stability or credit worthiness

    NOTES:

    1. Accreditation documents must be neatly fastened and arranged as formatted above.
    2. A Letter of Application (LOA) must be prepared and attached on the front page.
    LOA must be written on official company letterhead and be duly signed by an authorized official of the company. Please clearly indicate what specific business interest does your firm intends to be accredited for with SKI. LOA shall be addressed to: · MR. CHITO S. CORTEZ - Chairman (SKI Supplier Accreditation Committee)
    3. Original Copy of submitted documents must be presented for authentication if Certified True Copy is not available.
    4. All attachments (annexes and statements) must be properly labeled with index tabs.
    5. The Accreditation Committee reserves the right to deny or reject supplier application.
    6. Processing period is two (2) weeks from date of submission (if documents are complete).
